Treat decay while it is small
A root canal is needed when the soft tissue inside a tooth is inflamed or infected beyond saving. The commonest cause is decay that has been left to travel through the enamel and dentine until it reaches the nerve. A small cavity is cleaned and filled. A large one may need a root canal and a crown.
That gap is the reason a dentist finds early decay worth treating even when it does not hurt. Our guide on filling vs root canal explains where the line falls, and the guide on how to stop tooth decay covers the daily habits.
The habits matter more than the products: brush twice a day with fluoride toothpaste, clean between teeth, and limit how often you snack on sugar, sweet tea or sticky food, as frequency matters more than quantity.
Signs not to ignore
A tooth often warns before it needs a root canal. Acting at the first stage can be the difference between a filling and root canal treatment. Pain that wakes you at night, or lingers after something hot or cold, is a sign the nerve is struggling.
| Sign | What it may mean | What to do |
|---|---|---|
| Pain on sweet, hot or cold that goes quickly | Early decay or exposed dentine. | Book a check-up soon. A filling may be enough. |
| Pain that lingers after hot or cold | The nerve is inflamed. | See a dentist this week. |
| Pain when biting or a tender tooth | A crack, a high filling or infection at the root. | See a dentist soon. |
| Pain at night or throbbing | Nerve damage or infection. | Get seen promptly. |
| A tooth turning grey or a pimple on the gum | The nerve may have died. | See a dentist. Our guide on gum boils explains more. |
General warning signs. Only a dentist can tell which applies to your tooth.
Protect teeth from cracks, grinding and injury
A cracked tooth lets bacteria reach the nerve even without decay. Avoid biting ice, hard sweets and packaging, and treat a cracked or chipped tooth early. Our page on a broken tooth explains what to do.
If you grind at night, a night guard protects against wear and cracks, and our guide on a mouth guard for teeth grinding explains who needs one. If you play contact sport such as cricket with a hard ball, kabaddi or football, a fitted mouthguard guards against knocks that can kill a nerve.
Check-ups, and what cannot be prevented
A check-up and X-ray finds decay between teeth and under old fillings before it hurts. Our guide on how often to see a dentist covers timing.
Not every root canal can be avoided. A hard knock, a deep old filling or a crack can damage the nerve whatever you do, and a tooth that is already infected needs treatment. If you are told you need one, the root canal treatment page explains what to expect. Seek care the same day for facial swelling or fever, and go to a hospital emergency department if breathing or swallowing is affected.
Common questions
Can a root canal be avoided once a tooth hurts?
Sometimes. If the pain is from a shallow cavity, a filling may be enough. If the nerve is already inflamed or infected, the choice is root canal treatment or removing the tooth. A dentist can tell after an examination and X-ray.
Can a tooth heal without a root canal?
A tooth does not heal decay on its own. Early enamel changes can be reversed with good habits, but a cavity that has reached the nerve cannot. Pain that fades usually means the nerve has died, not that the tooth has recovered.
Does brushing prevent root canals?
It lowers the chance by preventing decay, but only when it is done twice a day with fluoride toothpaste along with cleaning between teeth and fewer sugary snacks. It does not protect against cracks or injury.
Is a mouthguard really needed for sport?
If you play a contact sport or one with a hard ball, a fitted mouthguard reduces the risk of broken teeth and nerve damage. A dentist can advise on one that fits properly.
