Zirconia vs Metal-Ceramic Crown: Which Should You Pick?

A metal-ceramic crown, also called PFM, has a metal shell fitted over the tooth with porcelain fused onto its outer surface. The metal gives strength and the porcelain gives appearance. It has been the workhorse of crown dentistry for decades.

A zirconia crown is solid zirconium dioxide, a ceramic strong enough not to need a metal substructure. It is milled from a block by computer-controlled machining rather than hand-built.

This is the decisive practical difference and it is rarely explained at the point of decision. In a metal-ceramic crown the metal shell extends to the crown margin at the gum. While the gum sits high, it is hidden. Gums recede with age, and when they do, a thin grey line appears at the edge of the crown.

On a molar nobody sees it. On an upper front tooth it is the reason many crowns get replaced at ten years despite being clinically sound. Zirconia has no metal anywhere in it and cannot do this.

Both are strong enough for molars. Monolithic zirconia is exceptionally fracture-resistant and is the material of choice for patients who grind. Metal-ceramic is also robust, but its known failure mode is the porcelain chipping off the metal, which cannot be repaired properly and means remaking the crown.

One caution applies to zirconia: it is hard, and a rough or unpolished zirconia surface can wear the opposing natural tooth. Properly glazed and polished, this is not a practical problem, but it is a reason to care which laboratory made it.

For a front tooth, lithium disilicate, commonly sold as E-max, is the best of the three. It transmits and scatters light closest to natural enamel, which is what makes a crown read as a tooth rather than as a crown.

Layered zirconia is next, with a zirconia core and porcelain built onto the visible surface. Monolithic zirconia is slightly more opaque and is excellent for back teeth. Metal-ceramic is last, because the metal core blocks light entirely, giving the tooth a flat, dead quality alongside its natural neighbours.

Back teeth: monolithic zirconia. Strength where it is needed, no metal line, and the appearance question barely applies.

Front teeth: lithium disilicate, or layered zirconia if the tooth underneath is discoloured and needs masking.

On a tight budget, or on a lower back molar that will never be visible: metal-ceramic remains a perfectly serviceable crown and there is no reason to feel short-changed by it.
