This is a different problem from early soreness
Tenderness in the first days or weeks is expected and is covered in does a root canal hurt. This page is about pain that appears after the tooth had settled: months or years later, or after a long comfortable period.
The nerve is gone, so the tooth itself cannot feel pain. What hurts is the tissue around the root: the ligament and the bone. Pain, tenderness to bite, a pimple on the gum or swelling means the area around the root is inflamed, almost always because bacteria are back or the tooth has been damaged.
The usual causes
Leakage. A cracked, worn or missing filling or crown lets saliva and bacteria seep back down the canals. This is the commonest cause, and it is why delaying the crown matters. See do I need a crown after a root canal.
Persistent infection. Canals can have branches or extra canals that cleaning did not reach, and a few bacteria can survive in these spaces. They cause symptoms slowly.
A crack or fracture. A root-treated tooth is brittle. A vertical root fracture is hard to see early and can mimic reinfection. It often cannot be treated and may mean extraction.
New decay under the crown or at its edge, gum disease around the tooth, and a high bite from grinding can also produce pain that feels like the root canal failing.
How it is diagnosed
The dentist taps and probes the tooth, checks the gum for a sinus opening or deep pockets, tests the bite, and compares a new X-ray with the earlier one. A dark shadow at the root tip that has grown, or a new one, points to infection. Sometimes a 3D scan is needed to see an extra canal or a crack.
Not every ache is the root canal. Pain from a neighbouring tooth, a sinus problem or jaw muscle strain can be felt in the treated tooth. This is why the tooth should be examined and not assumed to have failed.
What can be done
If the seal has leaked but the root is sound, the tooth is usually re-treated: the old filling is removed, the canals are cleaned again and re-sealed, and a new crown is placed. A re-treatment typically costs ₹7,000 to ₹15,000 in Lucknow in 2026, as listed on our root canal cost in Lucknow page.
If infection persists after re-treatment, a small surgical procedure at the root tip is sometimes an option. If the root is cracked, or too little tooth remains, extraction and replacement is the honest answer, and root canal vs implant explains how that decision is made.
Do not wait for the pain to pass. Swelling of the face or gum, or a fever, needs care the same day, and a hospital emergency department if breathing or swallowing is affected. Our root canal treatment page covers the original procedure.
Common questions
Is it normal for a root canal tooth to hurt after 5 or 10 years?
No, it is not expected. A comfortable root-treated tooth that begins to hurt has usually developed a problem such as reinfection, a crack or decay. Many causes are treatable, and the earlier it is examined the better the chance of saving the tooth.
Can I feel pain in a tooth that has no nerve?
The tooth itself cannot, but the ligament and bone around its root can. Inflammation or infection there causes pain on biting, tenderness, a pimple on the gum or swelling, even though the nerve inside the tooth was removed.
Can a root canal be redone?
Often, yes. Re-treatment removes the old filling, cleans and seals the canals again, and many teeth can then be restored. Success depends on the cause, and teeth with root fractures usually cannot be saved.
What does a gum pimple near a root canal tooth mean?
It usually means infection at the root tip draining through a small channel in the gum. It may be painless, which can be misleading. Have the tooth examined soon, because the infection will not clear without treatment.
Will antibiotics fix a painful old root canal tooth?
Not on their own. The source of infection is inside the tooth or the root, where antibiotics reach poorly, so the pain returns once the course ends. They are used only when a dentist advises, alongside treatment of the tooth itself.
