Where the six-month figure comes from
It is a convention rather than a finding. It has been the default for decades because it is a reasonable middle for an average-risk adult, and because it is easy to remember.
The evidence supports individualising it. A low-risk adult with no decay history, healthy gums and good cleaning gains little from six-monthly attendance and could reasonably go longer. A high-risk patient gains a great deal from attending more often, and six months is too long for them.
Who should attend every three to four months
Anyone treated for periodontal disease. This is the clearest case. Periodontal disease is controlled, not cured, and the recall interval is what protects the bone you have left. Reverting to six-monthly after treatment is a common way to lose the result.
Smokers. Deposits reform faster, healing is impaired, and bleeding, the usual early warning sign, is suppressed by nicotine constricting the gum vessels. Smokers get less warning and more disease.
People with diabetes, particularly where control is imperfect. The relationship runs both ways: gum disease worsens glycaemic control and poor control worsens gum disease.
Anyone with implants. Peri-implantitis is the leading cause of late implant loss and progresses faster around an implant than gum disease does around a natural tooth.
Anyone with a dry mouth, from medication or otherwise. Decay progresses unusually fast without saliva.
What the appointment is actually for
The cleaning is the visible part and the smaller part. The examination is the point: checking for decay between teeth where you cannot see, measuring gum pockets, looking at existing fillings and crowns for failing margins, and screening the soft tissues for anything that should not be there.
That last one matters more in Uttar Pradesh than the national average, because tobacco and areca nut use is high, and oral cancer found early has a very different outcome from oral cancer found late. A three-week-old ulcer that has been ignored is the presentation nobody wants.
The economic case, plainly
Decay found before it reaches the nerve is a filling at ₹500 to ₹3,000. The same decay found once it has reached the pulp is a root canal at ₹3,000 to ₹12,000 plus a crown at ₹3,000 to ₹25,000.
Gum inflammation found at the bleeding stage is a scaling at ₹800 to ₹2,500 and reverses within two weeks. The same disease found once bone has been lost is a lifelong management problem, and the bone does not come back.
That is the whole argument, and it does not depend on any claim about how good the dentistry is. Attending is cheaper than not attending.
Common questions
Do I need a dental check-up if nothing hurts?
Yes, and that is precisely the point. Decay does not hurt until it reaches the nerve, and gum disease does not hurt until the bone supporting the teeth is already lost. The problems worth catching are all painless at the stage where they are cheap to fix.
How often should I get my teeth cleaned?
Every six months for most people, and every three to four months if you smoke, have diabetes, have been treated for gum disease, or have implants. Scaling costs ₹800 to ₹2,500 in Lucknow and is the highest-return appointment in dentistry.
Is it bad if I have not been to a dentist in years?
It is common, and it is not a reason to keep not going. Expect the first appointment to be an examination, X-rays and a longer cleaning, and expect a written plan sequenced by urgency rather than everything at once. What you should not expect is a lecture.
How often should children see a dentist?
Every six months from the time the first teeth come through, and a first orthodontic assessment at around age seven. That assessment is not a recommendation to start treatment; it catches crossbites and jaw discrepancies at the stage where growth can still be used to help correct them.
