Two different lifespans
An implant is two things with two different life expectancies, and conflating them is why the answers you find online vary so widely.
The fixture, the titanium screw in the bone, is not subject to decay and does not wear. Long-term studies report survival above ninety percent at ten years and it frequently remains in service far longer.
The crown is subject to chewing forces, ceramic fatigue and the occasional fracture, and typically needs replacing at ten to fifteen years. Replacing a crown on an existing implant is a comparatively small job and does not disturb the fixture.
What actually ends an implant early
Peri-implantitis. This is gum and bone inflammation around the implant, caused by bacterial plaque in exactly the way gum disease affects natural teeth. It is the leading cause of late implant loss and it is largely preventable.
Smoking. It is the single strongest modifiable risk factor, impairing both the initial healing and the long-term stability of the bone around the fixture.
Uncontrolled diabetes. Poorly controlled blood glucose impairs healing and raises infection risk. Well-controlled diabetes is not a barrier to implant treatment.
Untreated grinding. Bruxism overloads the implant. A natural tooth has a ligament that cushions force; an implant is fused directly to bone and has no such buffer. A night guard is often part of the plan for this reason.
Poor initial placement. An implant placed without adequate bone, or angled so the crown loads it unfavourably, carries its problem from day one. This is what CBCT planning is for.
What you control
Clean around it daily, including under the crown where it meets the gum, using interdental brushes or a water flosser. Attend maintenance visits so deposits below the gum line are removed before they cause bone loss. Stop smoking, which improves the odds more than any other single change. Wear the night guard if you grind. Report any bleeding, tenderness or looseness early, because peri-implantitis caught early is manageable and caught late is not.
On lifetime guarantees
Some clinics advertise a lifetime warranty on implants. Read what it covers. It usually covers the fixture, which rarely fails on its own, and excludes the crown, which is the part that predictably needs replacing, and is typically void if you smoke or miss maintenance appointments. A guarantee is not a substitute for asking which implant system is being used and whether matching components will still be available in fifteen years.

What is the failure rate of dental implants?
Roughly five to ten percent over ten years in general populations, concentrated in smokers, patients with uncontrolled diabetes, and cases with inadequate bone or untreated gum disease. Early failure in the first months usually reflects a problem with integration; late failure is usually peri-implantitis.
Can a dental implant get decay?
No. Titanium and ceramic do not decay. This is why implants cannot get cavities, and it is also why people underestimate the risk: the failure mode moved from the tooth to the gum, and gum disease around an implant progresses faster than around a natural tooth.
Can a failed implant be replaced?
Usually yes. The failed fixture is removed, the site is often grafted, and after healing a new implant can be placed. It adds cost and several months, which is why prevention through cleaning and maintenance is worth more than it sounds.
