When Should a Child First See a Dentist?

Short answer

By the first birthday, or within six months of the first tooth appearing, whichever comes first. The first visit is mostly about examining, advising on feeding and cleaning, and letting the child get used to the surroundings. Starting early, before anything hurts, is the single most effective way to avoid producing an adult who is frightened of dentists.

Why so early, when they will lose those teeth anyway

The first visit is not really a dental examination in the adult sense. It is a prevention appointment: checking development, discussing bottle and night feeding, showing you how to clean a small child's teeth, and advising on fluoride.

It also establishes the dentist as a place where nothing bad happens. A child whose first visit occurs at age five with toothache learns that dentists are associated with pain. A child who has been coming since infancy for two-minute check-ups does not. This difference persists into adulthood and it shapes whether they attend for the rest of their lives.

Milk teeth matter more than people think

They hold the space for the permanent teeth. A milk molar lost early lets the teeth behind drift forward, and the permanent tooth that was going to occupy that space finds it gone. That is a straightforward route into orthodontic treatment that was avoidable.

They guide the permanent teeth into position. They are needed for chewing and for speech development. And infection in a milk tooth sits directly above the developing permanent tooth, which can damage its enamel formation permanently.

The phrase they are only baby teeth is the most expensive sentence in paediatric dentistry.

The things that actually cause decay in small children

Bottles and sippy cups at night containing anything other than water. Milk, juice and sweetened drinks pooling around the upper front teeth overnight, when saliva flow is at its lowest, cause a characteristic and rapid pattern of decay.

Frequency rather than quantity. A chocolate eaten in one sitting is less damaging than the same chocolate eaten in ten small pieces across a day, because each exposure restarts the acid attack. This surprises most parents and it is the single most useful thing to change.

Medicines with sugar taken at bedtime, particularly for children on long-term medication.

Not cleaning until the child can do it themselves. Children lack the manual dexterity to brush effectively until around age seven or eight. An adult should brush or supervise until then, whatever the protest.

How to prepare a child without making it worse

Keep the language neutral. Do not say it will not hurt, because you have introduced the word hurt and the possibility with it. Say the dentist is going to count your teeth.

Never use the dentist as a threat for not brushing. It is common and it is directly counterproductive.

Do not transmit your own anxiety. Children read a parent's tension precisely. If you are frightened of dentists, it is worth having the other parent bring them, or saying so quietly to the clinic beforehand.

Book a morning appointment when the child is rested, and go for a check-up when nothing is wrong before you ever need to go when something is.

What happens after the first visit

Six-monthly check-ups. Fluoride varnish applications, which are quick, painless and substantially reduce decay. Fissure sealants on permanent molars once they come through at around six, which seal the deep grooves where decay usually starts. And a first orthodontic assessment at around seven, which is a look rather than a treatment plan.

Tell the clinic beforehand so time can be set aside. Book a morning appointment, keep your language neutral, avoid promising it will not hurt, and never use the dentist as a threat for not brushing. A first visit that involves nothing more than sitting in the chair and counting teeth is a perfectly good use of an appointment.
