Do Cavities in Milk Teeth Need Treatment?

Short answer

Yes, in most cases. A cavity in a milk tooth does not stay put or fix itself. It grows, reaches the nerve, causes pain and can form an abscess that sits directly above the developing permanent tooth. Small cavities need a simple filling. Deeper ones need a pulpectomy or a crown, and only a tooth that cannot be saved is removed.

Why a cavity in a tooth that will fall out still matters

The common reasoning is that the tooth is temporary, so the cavity is temporary too. It does not work that way. Milk tooth enamel is thinner than adult enamel, so decay reaches the nerve far faster, often within months rather than years.

Some milk teeth are in the mouth for a long time. The back molars do not fall until about 10 to 12 years of age, so a child with decay in one of them has to chew on it for most of a decade if nothing is done.

Once the nerve is infected, the infection can drain into the gum above the tooth. That is where the permanent tooth is forming, and it can mark its enamel for life. Our guide on a child's first dental visit covers why milk teeth are worth protecting in general. This page is about what to do once decay has actually started.

How to spot it early

In the earliest stage, a cavity is not a hole. It is a chalky white or dull band along the gum line of the upper front teeth, or a rough patch on a back tooth. At this stage it can sometimes be reversed or held with fluoride and better cleaning, which is why a check-up before anything hurts is worth more than one after.

Later it turns yellow, brown or black, and a visible hole appears. Parents often report a child who avoids chewing on one side, cries at cold drinks, or has bad breath that does not go with brushing.

Pain is a late sign. Many children with large cavities say nothing at all, because they do not know that a tooth is not supposed to feel like that. If you can see a dark spot, assume it needs looking at.

What treatment depends on

The choice depends on how deep the decay is, how many teeth are involved, which tooth it is, how long until it would naturally fall out, and how well the child can cooperate in the chair.

A small cavity that has not reached the nerve is cleaned out and filled. This is the simplest case and the most comfortable for the child.

A deeper cavity that has reached the nerve usually needs a pulpectomy, the milk tooth version of a root canal, followed by a filling or a crown. A tooth with a large amount of breakdown may be better protected with a crown, because a big filling in a thin milk tooth tends to fail.

A tooth that is badly broken down, abscessed with no way of saving it, or very close to falling out is removed. If it is a back tooth with years left, the dentist will usually place a space maintainer so the neighbouring teeth do not drift into the gap. In Lucknow in 2026, a milk tooth extraction runs about ₹300 to ₹1,000 and a filling about ₹500 to ₹3,000. See the tooth extraction cost page for how those ranges are built.

What causes it in small children, and what stops it coming back

The usual drivers are sweetened drinks or milk in a bottle at night, frequent snacking across the day, sugary medicines at bedtime, and brushing that the child does alone before they have the dexterity for it.

Frequency matters more than quantity. Each sugary exposure restarts an acid attack on the enamel for roughly half an hour, so grazing is more damaging than one dessert after a meal.

Brush twice a day with a fluoride toothpaste, and brush the last time at night with nothing but water afterwards. An adult should brush or finish the job until the child is about seven or eight. Our guide on fluoride treatment for children covers toothpaste amounts and professional fluoride, and pit and fissure sealants cover the deep grooves of the back teeth.

A child who has had one cavity is at higher risk of the next, so the follow-up plan matters as much as the first filling. The wider picture of children's care is on our kids dentistry page.

When it cannot wait

Swelling of the gum or face, a pimple-like bump on the gum, fever with tooth pain, or a child who will not eat or sleep because of pain should be seen the same day. A swelling that spreads across the face, or any difficulty swallowing or breathing, is a hospital emergency.

Do not give antibiotics left over from another illness. They do not fix the tooth and they can hide the problem. Paracetamol as directed on the label for the child's weight and age can ease pain until the appointment. Ask a doctor or pharmacist first if the child has any other condition.

Common questions

Can a cavity in a milk tooth heal on its own?

No. Enamel cannot rebuild a hole once one has formed. The very earliest chalky white stage, before any hole appears, can sometimes be held with fluoride and better cleaning. Once the surface has broken down, the cavity only gets deeper without treatment.

Is it safe to leave a cavity until the milk tooth falls out?

Usually not. Decay in a milk tooth can reach the nerve within months and cause pain or an abscess. Infection above the tooth can also affect the permanent tooth forming underneath. The only exception is a tooth that is already very loose and about to fall.

Will my child need an injection for a milk tooth filling?

Small cavities that stay shallow are often treated with little or no anaesthetic. Deeper decay is treated under local anaesthetic so the child feels no pain. The dentist decides this tooth by tooth, and explaining what will happen in calm, plain words helps most children settle.

How much does a milk tooth filling cost in Lucknow?

A milk tooth filling is broadly comparable to an adult filling, at ₹500 to ₹3,000 in Lucknow in 2026, depending on the size of the cavity and the material used. A milk tooth extraction is about ₹300 to ₹1,000.

What if my child has cavities in several teeth?

Several cavities usually mean the cause, often night feeding, frequent sweet snacks or limited brushing, needs to change as well as the teeth being treated. The dentist will plan the order of treatment and a prevention routine, so the problem does not simply return.
