Loose Tooth: Causes in Children and Adults, and When It Is Urgent

Short answer

In a child of about six to twelve, a loose tooth is usually a milk tooth getting ready to fall out, which is normal. In an adult, a loose permanent tooth is never normal. The usual causes are gum disease, injury or heavy grinding, and it needs a dentist soon, because early treatment can often save the tooth.

Loose tooth in a child

Milk teeth loosen when the permanent tooth beneath pushes up and the root of the milk tooth dissolves. Most children start losing front teeth at about six, and the process runs until around twelve. A wobbly tooth with no pain, in a child of this age, is normal. Let it fall out by itself. Encourage gentle wiggling with a clean tongue or fingers, but do not pull a tooth that is only slightly loose. You can read the usual order in our guide on milk teeth falling age.

A milk tooth that is loose in a younger child, or after a knock, is different. Injury can loosen a tooth without it being ready to fall, so a dentist should check. A loose tooth with pain, a swollen gum or pus can be an infection under the milk tooth and needs a dentist.

Loose tooth in an adult

Permanent teeth should be firm for life. A tooth that starts to move is a sign that something has damaged what holds it. The common causes are below.

CauseWhat happensOther signs
Gum disease (periodontitis)Infection destroys the bone and fibres that anchor the tooth.Bleeding gums, bad breath, receding gums, pus, gaps opening between teeth.
InjuryA blow loosens or shifts the tooth in its socket.Pain on biting, bleeding from the gum, change in the tooth's position.
Teeth grinding or clenchingHeavy forces strain the supporting tissues over time.Worn teeth, jaw ache, morning headache.
Infection at the rootAn abscess around the root can loosen the tooth.Throbbing pain, a gum boil, swelling.
Hormonal changesPregnancy can make gums more inflamed and teeth feel looser.Red, swollen, bleeding gums.

General information on common causes of loose adult teeth, not a diagnosis. Only an examination and X-ray can tell the cause.

Can a loose tooth tighten up again

Often yes, if the cause is treated early. A tooth loosened by a minor injury may settle in a few weeks if it is left alone and not bitten on, and sometimes needs a splint to hold it still while it heals. A tooth loosened by gum disease may firm up after the gums are cleaned and the infection is controlled, although bone already lost does not grow back on its own. See our guides on stages of gum disease and pyorrhea to see how it develops.

While you wait for your appointment, keep the area clean with gentle brushing, avoid biting hard foods on that tooth and do not push it with your tongue or fingers. Smoking and chewing tobacco or gutka make gum disease worse and slow healing.

When a loose tooth is urgent

See a dentist the same day or the next day if an adult tooth is loose after a knock, if the tooth has moved out of position, or if there is pain, pus or a swollen gum. A tooth that is completely knocked out needs help within the hour, as described in our guide on an avulsed tooth.

Spreading facial swelling, fever, or difficulty swallowing or breathing needs same-day care, and a hospital emergency if breathing or swallowing is affected. A loose tooth with no pain should still be seen within a week or two, because gum disease is often painless until late. The gum treatment page describes what is usually involved.

Common questions

Should I pull out my child's loose tooth?

If the tooth is hanging by a thread and comes out with a gentle wiggle, that is fine. Do not force a tooth that is firmly attached, since it can leave part of the root behind and cause pain and bleeding.

Is a loose tooth in an adult always gum disease?

No, injury, grinding and infection at the root can also loosen a tooth. Gum disease is the most common cause, so it is looked for first.

Can a loose tooth be saved?

Often, if it is seen early. Treating the gum disease, splinting the tooth after injury or reducing grinding can all help it firm up. A tooth with severe bone loss may not be saveable.

My toddler has a loose front tooth after a fall. What should I do?

See a dentist the same day. A milk tooth that has been knocked loose may need to be removed or monitored to protect the permanent tooth underneath.
