What causes it
The back of the tongue
This is the single largest source and the most commonly missed. The rear third of the tongue has a deeply grooved surface that harbours bacteria producing volatile sulphur compounds. It is the part of the mouth people clean least, because reaching it triggers the gag reflex.
Gum disease
Periodontal pockets are oxygen-poor spaces ideal for the anaerobic bacteria that produce the worst-smelling compounds. Bad breath that persists despite good tongue and tooth cleaning is gum disease until proven otherwise.
Dry mouth
Saliva continuously washes away bacteria and food debris. Anything reducing it, mouth breathing, sleeping with the mouth open, many common medications, dehydration, makes breath worse. This is why morning breath is universal and not a sign of disease.
Decay, food traps and failing restorations
A cavity, an overhanging filling edge or a poorly fitting crown creates a space that cannot be cleaned. Food stagnates there and the smell is localised and persistent.
Tonsil stones
Small calcified deposits in the tonsillar crypts, often visible as white specks. They produce a distinctly foul smell and are a common reason bad breath persists despite genuinely excellent dental hygiene.
Non-oral causes
The remaining fifteen percent. Sinus and post-nasal drip, reflux, poorly controlled diabetes, and liver or kidney disease each produce characteristic breath. These are worth investigating only once the mouth has been properly examined and treated, because that is where the odds sit.
What helps before you are seen
Clean the back of the tongue, properly
A tongue scraper, drawn from as far back as you can tolerate towards the tip, several strokes, once or twice a day. This single change resolves more cases than anything else. The gagging reduces within a week or two as you adapt.
Clean between the teeth daily
A toothbrush reaches roughly sixty percent of the tooth surface. The surfaces it misses are exactly where anaerobic bacteria sit. Floss or interdental brushes are not optional here.
Keep the mouth wet
Water through the day, and sugar-free gum to stimulate saliva. Avoid alcohol-containing mouthwashes, which dry the mouth and make the underlying problem worse after the initial masking effect wears off.
Test it honestly
Cupping your hands over your mouth does not work; you are habituated to your own smell. Lick the inside of your wrist, let it dry for ten seconds and smell it, or simply ask someone you trust. Many people convinced they have bad breath do not, and a smaller number who are certain they do not, do.
How it is treated
Examination to find the source
Establishing whether the source is the tongue, the gums, a specific tooth, or outside the mouth entirely is the whole job. Treating the wrong one is why people cycle through mouthwashes for years.
Scaling and gum treatment
Removing the deposits that anaerobic bacteria live in. Routine scaling costs ₹800 to ₹2,500 in Lucknow. Where pockets have formed, deep cleaning per quadrant runs ₹1,500 to ₹4,000.
Fixing food traps
Replacing an overhanging filling, treating decay, or correcting a poorly contoured crown removes the stagnation area. Often a single tooth is responsible.
Managing dry mouth
Reviewing medications with your doctor where they are the cause, saliva substitutes, and addressing mouth breathing. Dry mouth is manageable rather than curable, and managing it well makes a large difference.
Referral where the cause is not dental
If the mouth is genuinely healthy and the problem persists, that is the point to investigate sinus, reflux or metabolic causes, not before.
Bad breath: common questions
Why does my breath smell bad even though I brush twice a day?
Because brushing cleans teeth, and most bad breath comes from the back of the tongue and from between the teeth. A toothbrush reaches neither. Adding a tongue scraper and daily interdental cleaning resolves the majority of cases that brushing alone did not.
Does mouthwash cure bad breath?
No. It masks the smell for roughly twenty minutes and does nothing to the bacterial reservoirs producing it. Alcohol-containing mouthwashes make matters worse over time by drying the mouth, since saliva is one of your main defences against the odour.
How do I know if I have bad breath?
You cannot smell your own reliably, because you are habituated to it. Lick the inside of your wrist, let it dry ten seconds and smell it, or ask someone you trust to be honest. A dentist can also assess it directly.
Can bad breath be a sign of something serious?
Usually not. Around 85 percent of cases are oral in origin and treatable. The remainder can reflect sinus problems, reflux, poorly controlled diabetes, or liver and kidney disease. Those are worth pursuing after a dental examination has ruled out the far more likely causes.
