Mouth ulcers

Mouth Ulcer Vitamin Deficiency: B12, Folate, Iron and Zinc

Short answer

Low vitamin B12, folate and iron are the deficiencies most often linked to recurrent mouth ulcers, and zinc is sometimes involved. People on vegetarian diets are more likely to be low in B12. A blood test shows which, if any, is the cause. Supplements work best when chosen after testing and taken as directed on the label.

Which deficiencies are linked to mouth ulcers

Vitamin B12, folate (vitamin B9) and iron are the three that doctors check first in people who get ulcers repeatedly. They are all needed to renew the cells that line the mouth, so a shortage can leave the lining fragile. Zinc has also been linked, though less strongly.

A deficiency is only one possible cause of ulcers. Most people with an occasional ulcer are not deficient, and a deficiency can be present without any ulcers. The pattern to notice is ulcers that return again and again, sometimes with a smooth, sore tongue, cracked mouth corners, tiredness or paleness.

NutrientOther signs that may go with itWho is more at risk
Vitamin B12Tiredness, smooth sore tongue, tingling in hands or feetPeople on vegetarian or vegan diets, older adults, some stomach or gut conditions, some long-term medicines
FolateTiredness, sore tongue, mouth ulcersPoor intake of leafy vegetables and pulses, pregnancy, alcohol use
IronTiredness, paleness, sore corners of the mouth, hair lossWomen with heavy periods, pregnancy, poor diet, blood loss
ZincSlow wound healing, taste changePoor diet, some gut conditions

General guide to nutrients linked to recurrent mouth ulcers. It is not a diagnosis; a blood test is needed.

Test first rather than guessing

A blood test for haemoglobin, B12, folate and ferritin (iron stores) takes the guesswork out. Starting a vitamin on a hunch can hide the problem, and the wrong one will not help. Too much of some nutrients can also cause harm, particularly iron.

Ask a dentist or doctor about a blood test if ulcers appear several times a year, last longer than usual, or come with tiredness. Our guide on which doctor to see for a mouth ulcer explains the options, and aphthous mouth ulcers covers other triggers worth checking.

Food, supplements and Indian diets

B12 is found in animal foods, including milk, curd, paneer and eggs, but a strict vegetarian diet with little dairy can leave stores low. Folate comes from green leafy vegetables, dal, rajma and chana. Iron comes from leafy greens, pulses, jaggery and meat; vitamin C from fruit helps it absorb, while tea taken with meals reduces absorption.

If a test shows a deficiency, a doctor chooses the supplement and the duration. If you buy a multivitamin on your own, take it as directed on the label, and ask a pharmacist or doctor first if you are pregnant, take other medicines or have kidney or liver disease. Supplements do not replace a diagnosis.

When ulcers need more than vitamins

Correcting a deficiency often reduces ulcers but is not guaranteed to stop them. A tongue that is sore or changed is worth a look; our guide on inflammation of the tongue explains why. Cracks at the mouth corners are covered in the guide on angular cheilitis.

An ulcer that has not healed within three weeks needs examining whatever the blood test says. See the mouth ulcers page for the full list of warning signs.

Common questions

Which vitamin deficiency causes mouth ulcers most often?

Vitamin B12, folate and iron are the usual ones checked. Zinc is sometimes involved. A blood test is the only way to know whether any of them is the cause in your case.

Should I take vitamin B12 tablets for mouth ulcers?

It is better to test first. If you are low, a doctor can choose the right supplement. If you buy one yourself, follow the label and ask a pharmacist or doctor first if you take other medicines or have health conditions.

Can a vegetarian diet cause mouth ulcers?

Not by itself, but a strict vegetarian diet with little dairy can leave vitamin B12 low, and that can be one cause of recurrent ulcers. Milk, curd and paneer help. A blood test shows whether it applies to you.

How long after correcting a deficiency do ulcers stop?

It varies. Some people notice fewer ulcers over weeks to months once levels are corrected. If ulcers continue, other causes such as stress, a sharp tooth or a medical condition should be checked.
