Fordyce Granules in Mouth: Harmless Spots or Something to Check?

Short answer

Fordyce granules, also called Fordyce spots, are small, painless yellow-white dots on the lips and inner cheeks. They are normal oil glands that sit close to the surface, not infectious and not cancerous. They need no treatment. A spot that changes, grows, hurts, bleeds or does not rub off like the others is worth showing to a dentist.

What Fordyce granules are

Fordyce granules are enlarged sebaceous glands, the same oil glands that sit in the skin. In the mouth, they show as clusters of tiny, slightly raised, pale yellow or white dots, usually on the inner cheeks and on the edges of the lips. Many people have them without noticing, and they often become more visible in adulthood.

They are not an infection, an allergy or a sign of poor hygiene. They are not passed on to anyone else, they do not cause pain, and they do not turn into anything harmful.

Do they need treatment?

No. Because they are a normal feature, there is nothing to cure. Creams, rinses and home remedies do not make them go away, and squeezing or scraping at them only irritates the tissue. If a person is bothered by the look, a dentist or skin specialist can discuss options, but this is a cosmetic choice rather than a medical need.

Brush and floss as usual. The spots are not a reason to change anything you do.

How to tell them from spots that do need a look

Fordyce granules are tiny, symmetrical on both sides, painless, and have usually been present for a long time. Spots that are new, single, painful, bleeding, ulcerated or growing are different and should be seen. A white patch that does not rub off and lasts over three weeks needs examining; our guide on white patches in the mouth explains the common causes.

On the tongue, small raised bumps are usually the normal taste buds; our guide on bumps on the back of the tongue explains which are normal. Cold sores and thrush look different from Fordyce granules and are covered on the mouth ulcers page.

Common questions

Are Fordyce granules an STD or contagious?

No. They are normal oil glands, not an infection, and they cannot be caught or passed on by kissing or any other contact.

Can Fordyce granules be removed?

They do not need removing. If someone is bothered by their look, a dentist or skin specialist can talk through options, but they are harmless and can be left alone. Do not pick at them.

Why have my Fordyce spots become more obvious?

They commonly become more visible in the teens and adulthood and can look more prominent when the lips are stretched or when you are dry. Becoming more noticeable alone is not a sign of anything wrong.

When should I worry about a spot in my mouth?

If it is single and new, painful, bleeding, growing, or a patch that does not rub off and has lasted more than three weeks, have it examined. That is true even if it does not hurt.
