What is oral submucous fibrosis?
In OSMF the tissue just under the lining of the cheeks, palate and lips slowly turns fibrous and tight, like scar tissue. It starts as a burning feeling, especially with spicy food, and a dry mouth. Over time the cheeks feel stiff, bands can be felt inside the mouth, the lining looks pale or marble-like, and it becomes difficult to open the mouth fully.
It is common in South Asia, where areca nut chewing is widespread. Because the changes are gradual, many people do not notice until opening the mouth is already limited. Our guide on mouth cancer first stage lists other early changes to watch for.
What causes OSMF?
The main cause is chewing areca nut, in any form: raw supari, paan, and ready-made pouches of gutka and paan masala, which concentrate it. The more often and the longer someone chews, the greater the risk, and young people who start early can develop it within a few years. Chilli and nutritional shortages may play a part, but areca nut is the key factor.
OSMF is called a potentially malignant condition, meaning a proportion of cases can later turn into mouth cancer. Using tobacco as well adds to that risk, as described in smoking and mouth cancer. Areca nut is not a safe alternative to tobacco.
| Stage | What a person may notice |
|---|---|
| Early | Burning on eating spicy food, dry mouth, small blisters or ulcers |
| Developing | Cheeks feel tight, mouth lining looks pale, bands can be felt |
| Advanced | Hard to open the mouth, trouble eating, speaking or cleaning teeth |

OSMF treatment and mouth opening exercises
Stopping areca nut, supari, gutka, paan masala and tobacco is the single most important step. Without that, the stiffening tends to keep progressing. Stopping may ease the burning and prevents further damage, although fibrosis that has already formed does not fully reverse. If quitting is hard, ask a doctor about quit support.
The rest depends on how far it has gone, and is decided by the treating dentist, doctor or specialist. This can include medicines, and mouth opening exercises. Those exercises are prescribed and taught by the treating clinician, so do not start a routine on your own, as stretching the wrong way can hurt the tissue. Advanced cases may need a specialist's care.
When to get it checked
Get a dentist or doctor to look if you chew areca nut, supari, gutka or paan masala and notice burning with food, a tight cheek, bands inside the mouth, or that you cannot open as wide as before. Do not wait for pain.
A white or red patch, an ulcer or a lump that has not healed in about two to three weeks also needs checking by a dentist or doctor. Read how a mouth ulcer differs from cancer and which dentist or doctor to see for mouth cancer. If swallowing or breathing becomes difficult, go to a hospital emergency department.
Common questions
Is OSMF the same as mouth cancer?
No. OSMF is a potentially malignant condition, which means it can turn into cancer in some people over time, but it is not cancer itself. It still needs to be checked regularly.
Can OSMF be cured?
Stopping areca nut stops it getting worse and can ease burning, but stiffness that has already formed does not fully reverse. Earlier is better, so see a dentist or doctor soon.
What are the first signs of OSMF?
Burning in the mouth with spicy food, a dry mouth, a pale lining, and a tight feeling in the cheeks. Reduced mouth opening comes later.
Who prescribes mouth opening exercises for OSMF?
The treating dentist, doctor or specialist decides whether exercises suit you and teaches the right method. Do not start a home routine without that advice.
