Mouth cancer

Difference Between Mouth Ulcer and Cancer: How They Compare

Short answer

An ordinary mouth ulcer is usually painful, heals within about two weeks and tends to come and go. A cancer-related sore is often painless and does not heal, and may have a hard or raised edge. The two can look alike, so any ulcer lasting three weeks needs an examination rather than a guess.

Mouth ulcer or cancer: how they usually differ

Most mouth ulcers are the ordinary kind. They hurt, appear in the soft lining of the lip or cheek, and heal on their own, as our guide on aphthous mouth ulcers describes. A sore that is an early cancer behaves differently, mostly by not going away.

The table below compares what people commonly notice. It is a general guide to why persistence matters, not a test. Features overlap, and an ordinary ulcer can have some of the warning features, so the table cannot tell you which one you have.

FeatureOrdinary ulcerWarning sign
PainUsually sore, especially with spicy or salty foodOften painless in the early stage
Healing timeHeals within about two weeksStill present after three weeks, or slowly getting bigger
EdgesRound or oval with a red rimRaised, rolled or hard edges

RecurrenceComes back in new places, then healsStays in the same spot and does not heal

Habits and riskStress, a bite, a sharp edge, low iron or B12Tobacco in any form, areca nut, regular alcohol

General comparison for awareness only. It cannot tell you which one you have: any ulcer lasting three weeks needs examining.

Can a mouth ulcer cause cancer?

No. An ordinary ulcer does not turn into cancer. The worry comes from the other direction: an early cancer can look like an ulcer at first, so a sore that was never an ordinary ulcer is mistaken for one and left alone.

Long-standing irritation from a sharp tooth or a rough filling is worth fixing, but the reason is comfort and healing. It is the habits, such as tobacco and areca nut, that are linked to mouth cancer, as explained in our guide on smoking and mouth cancer.

Why you cannot tell by looking, and what to do

Even trained dentists and doctors cannot say from appearance alone whether a sore is harmless or not. That is why the rule rests on time, not on how it looks: any ulcer, red or white patch, or lump that has not healed within three weeks should be examined, painful or not. If you use tobacco or areca nut, have a new sore checked sooner.

At the visit, the dentist or doctor looks for a cause, such as a sharp edge, and may review it again after a short interval. If it still looks doubtful, a small sample is taken, which is the only way to know for sure. Our guide on how mouth cancer is diagnosed explains that step.

Common questions

Can a painful ulcer be cancer?

Pain is not a reliable guide either way. Early cancer is often painless, but some sores become sore, especially once they are infected or irritated. Judge by how long the sore has lasted, not by how much it hurts.

How long is too long for a mouth ulcer?

Three weeks. An ordinary ulcer should be well on the way to healing or gone by then. If it is still there, have it examined even if it is not bothering you.

Should I wait and see if it goes away on its own?

You can give an ordinary ulcer up to three weeks, and fix any obvious cause such as a sharp tooth in the meantime. After that, waiting longer has no benefit. If you chew tobacco or areca nut, do not wait the full three weeks.

Do gels and home remedies tell me whether it is serious?

No. A gel may ease soreness and make an ulcer more comfortable, but it does not show whether the sore is harmless. A sore that is not healing is not one to keep treating at home.
